Family Foster Care


The Family Foster Care program at Nazareth Children’s Home is designed to find stable,temporary, family style placements for children who are in need of out of home placement due toabuse, neglect and dependency. This program serves children from birth through the age of 21,male or female and often serves sibling groups of two or more. Nazareth Children’s Home works in conjunction with Departments of Social Services as they are the legal guardians for these children. This partnership assures that all services are secured for the children while they are in the Family Foster Care program.

Family Foster care parents receive a minimum of 30 hours of pre-service training. Nazareth Children’s Home utilizes the Group Preparation and Selection/Partnerships in Parenting (GPS/MAPP) training curriculum. This curriculum is that same training that Departments of Social Services use for their foster parents. All family foster care parents participate in the home study process, which includes participating in training, consultation visits, medical visits, fingerprints, fire inspections and a health and safety inspection. Nazareth Children’s Home submits all licensing materials to the Licensing Authority and they make the final decision regarding licensure.

Therapeutic Foster Care:

The Therapeutic foster care program at Nazareth Children’s home is designed to provide a structured family environment for children who have behavioral and emotional needs outside the scope of family foster care. This program services children from the age of 4 through 21, male or female. The children served must have a mental health diagnosis such as Oppositional Defiant Disorder or Attention Deficit Disorder. Nazareth Children’s Home works as part of a team with other community resources to assure that the child receives all appropriate services.

Therapeutic Foster Care parents receive a minimum of 40 hours of pre-service training. This includes the GPS/MAPP training along with 10 additional hours of “How to Become a Therapeutic Parent”. The Therapeutic Foster Care case manager provides weekly in home consultation as well as individualized training.

Preparation for Adult Living – Transitional Living Services

This program serves males and females between the ages 16 – 21. The purpose of the program is to prepare adolescents and young adults to be self-sufficient regardless of their future vocational and academic choices. However, Nazareth Children’s Home encourages and financially supports those who choose the path of higher education. There are many institutions of higher education in the surrounding area………Catawba College, Livingstone College, Pfeiffer University and Rowan Cabarrus Community College. Life skills training, community awareness and promotion of individual self-esteem is the basic responsibility of the program. Decision making skills, employment, money management and housing are also a major focus of the program. Programs of this type were conceived out of a need in the childcare profession to better prepare young people for the eventuality of life outside of the child welfare system. Our program is designed to offer low structure in a residential setting to youth who can benefit from being more self-reliant while raising the structure for those who are not ready for the higher level of responsibility that adults must face. The youth are involved in strength based assessments and formal and informal life skills teaching are geared toward the youth’s interests and individual Transitional Living plan.

Transitional Living Apartments are offered as the next level in the program, where residents are able to reside in an apartment setting. The purpose of the Transitional Living Program is to provide a place for supervised practice where individuals may develop the skills they have learned. This program male and females between the ages of 16 – 21 in their transition from supervised apartment living to living independently in a community setting. They are able to practice, under minimal supervision, the living skills taught to them in the Preparation for Adult Living Program. The youth will work with the program Case Manager, Apartment Manager and Resident Counselors to continue setting goals that will prepare them to live independently. All referrals are made by the Department of Social Services; as placement in the PAL Program is a prerequisite and the Transitional Living Apartments are considered to be an “extension” of the PAL Program, with a continuation of a positive youth development approach.

Transitional Care

Purpose… The Transitional Care Program is designed to provide effective and therapeutic care to children that have been removed from the custody of their family or caretaker often as the result of physical, emotional, sexual abuse, neglect or dependency. The Transitional Care Program provides skill building in a family style setting to youth that have significant deficits in social skills and/or behavioral controls. The program teaches youth appropriate ways of getting their needs met. The program utilizes the Teaching Family Model of Care to assist youth to change maladaptive behaviors that they may have used in the past to get needs met and replace them with appropriate skills. By doing so, the youth will be more likely to succeed in all settings. The Transitional Care Program’s goal is to assist youth in internalizing these positive behaviors so that they may be reunified with family, placed in a more traditional setting, and/or be able to maintain a their placement over time.

Target population… Transitional Care is designed to care for children who are between the ages of 5-18 years old, either male or female. The typical child is under 16 years old, as the children over 16 years old are referred to the Preparation for Adult Living Program (PAL). However, the Transitional Care Program is not restricted by age. The children must be able to attend public school and benefit from a group living environment.

Admissions… Referrals are accepted from County Departments of Social Services, Juvenile Justice and Mental Health. All residents must be in the custody of the Department of Social Services. A state application and required records of each child referral should be noted or faxed to the agency. The Transitional Care Social Worker will contact the person referring the child after the Admission Committee reviews the documents.

ACE Residential Level II Program

North Caroline has undergone major changes in its child welfare system. It has created a need for specialization in childcare facilities. Nazareth Children’s Home has targeted a specific population of North Carolina’s children and developed a program that specifically address deficit areas children may exhibit in their home environment, educational system, and the community. Residential Level II provides mental health services for those individuals who has documented struggles in the above environments and has a mental health diagnosis that helps to define the reasons for the socially unacceptable behaviors displayed. The program provides short-term therapeutic group home care for those individuals referred by the Community Support Mental Health Agencies. Along with the campus Case Manager, the family, Community Support, and the clients, the goal is to successfully reunite these individuals with their families equipped with the tools they need to better cope with challenges in their lives.

Referral-Referrals for the Residential Level II are received from the Community Support Mental Health Agencies. The majority of the clients will remain in the custody of their legal guardians. The Department of Services will be involved with some of the clients, but they will work with the mental health agencies in order to have a client referred to the program. The potential client must be a Medicaid recipient.

The Residential Level II Program is licensed to serve males ages 12-18. The length of stay is determined by how well the client meets his outlined goals defined on his Person Centered Plan. The Person Centered plan is formed by the client, his guardian(s), and the Community Support Case Manager.

Team Concept-Nazareth Children’s Home believes in the team concept of childcare. The teams in the program consist of twelve Direct Care Staff that are divided into teams of three and work twelve hour shifts. Our team also consists of a House Manager, Case Manger, and Program Director. We all work together to provide all clients with the skills and knowledge they need to be successful and productive citizens.

Day Treatment

New Bridge Academy is a Day Treatment program that provides behavioral support, therapeutic interventions, behavior modification and academic instruction in a unique structured learning environment for eligible middle school and elementary school students in the Rowan/Salisbury School District. The Day Treatment program is a Mental Health program that provides services to clients with a Mental Health diagnosis. The students find success through therapeutic group, behavior modification, individualized plans and hands on experiential learning. The program utilizes a positive based behavior modification model that is individualized for each student’s needs and focuses on developing social skills/behavioral controls needed to assist them in being more successful in the classroom, home and community setting. The program assists students with behavioral deficits that have been displayed in a traditional classroom setting replace them with appropriate skills so that they may return to the traditional setting and be successful. Each program is staffed with a Program Manager, trained in behavior modification that is a Qualified Mental Health Professional, a certified Exceptional Children’s Teacher and Behavioral Technicians as appropriate to meet mental health standards. Referrals are made through the Rowan/Salisbury School System, mental health providers, parents and other local agencies.
